Application for New Membership with Proficiency Certificate - PC
TERMS & CONDITIONS: PROCESSING OF APPLICATIONS: Applications which are incomplete due to insufficient data, lack of required materials and non payments will not be processed. These include Membership Form duly completed, Photo for the Membership Card and other necessary documents associated with the Proficiency in Golf Certificate.
Our Admin Staff will contact the Applicants to arrange for a PC Test. The processing time schedule for the Membership Card is about 1 week (7 working days) after the completion of your PC Test .
MEMBERSHIP PHOTO: You may submit your photos either by email to (firstname.lastname@example.org) or What’s App to (9665 0100). Photos submitted during the initial registration will be reused for future Renewals or Handicap Upgrades unless otherwise requested by the renewing member.
AGE GROUP: Membership Age Group is calculated based on the Current Year of Membership.
Junior - Age Group from 6 to 20 years old Adult - Age Group from 21 to 54 years old Senior - Age Group from 55 years old and above
DELIVERY: Upon completion, Cards will be mailed out to the respective Members via Sing Post. A fee of $30.00 is chargeable should you require the Membership Cards to be sent by Courier Service. (Local destinations only). Members may also collect their Card from our Office, directly.
REPRINTING: Please note that there will be a Reprint charge of $30.00 for cards with errors resulting from misspelt names submitted via the E-Application Form. Therefore please check for errors before submitting the form.
GOLFER’S INSURANCE: The golfer’s group insurance services are exclusively for our Members only. Members who have opted not to take our optional services are advised to be adequately insured.
HANDICAP UPGRADE: Members may opt to upgrade to a Handicap Index after they have submitted Score Cards from recently played golf games (5 X 18 Holes/ 10 X 9 Holes).
Please contact our Admin Staff to arrange for a Golf Workshop and a Handicap Test/ Courtesy Round. The processing time schedule for the Handicap Card is about 1 week (7 working days) after the completion of your Handicap Test/ Courtesy Round . One you have upgraded to a Handicap Index, the current PC will become invalid.
BYE LAWS AND CONSTITUTIONS: All Applicants upon acceptance of the Membership are governed by the Bye Laws of the Club stated in the Constitution. Applicants are kindly advised to read and understand the Bye Laws, Constitutions, Terms & Conditions stated in our Website, prior to the submission of this Membership Application.
DATA PROTECTION: By signing up for any products or services offered by us, you are deemed to have agreed & consented to the use of Personal Data for the purpose of receiving membership updates or interaction with us.

ONLINE PAYMENTS: Members who wish to pay online via Credit Cards (VISA, Master & Amex) may do so after submitting this form. Upon submission of this E-Form, Members would be directed to an Acknowledgement page. Please proceed to click on the link at the bottom of the Acknowledgment page.
CASH, CHEQUE or CREDIT CARD PAYMENTS: Cash, Cheque or Credit Card Payments are to be made at our Office. Cheques should be crossed, issued Payable to Golf Allianze Pte Ltd and mailed out to our Office Address: 33 Ubi Ave 3. Vertex Tower A #08-68. S(408868). Payments can also be made at any of our Authorised Payment Centres.
BANK TRANSFER: For all payments via Bank Transfers, a copy of the Transaction Slips/ Receipts must be emailed to email@example.com as proof of Payments.
